Questionnaire Help

To properly fill out the questionnaire online you must first locate your personal individual 6 digit juror identification number and your personal 5 digit electronic signature number.  These numbers are located on the front of the questionnaire.  The location of the 6 digit juror identification number is in the upper right hand corner of the questionnaire.  The five digit electronic signature number is located in the upper left hand corner under the web address.  They are in bold type. 

After you have entered your personal individual numbers, the login page will also ask you to enter your date of birth.   If your date of birth has been rejected, call the jury commission.  A reason for this error to occur would be if two people have the same first and last name.  Re: James Smith Sr. and James Smith Jr.. Our office would need to provide you with the right information as to whom the questionnaire belongs.

Once submitted the questionnaire may not be changed and/or corrected by you.


Summons Help

After being summoned for jury service:  To check the status of your correct time and date, you will need to locate your 6 digit juror identification number and your 5 digit electronic number.  Those numbers are located in the first paragraph on the body of your summons.  They are in bold type.  Enter each number in the proper space on the login page with your date or birth.  The next page will give you the day and time that you have been selected to appear.

Deferral:  Jurors will find a deferral button on the bottom of the appearance page.  If you have been selected to appear at a time that is inconvienient, you may ask for a ONE time deferral.  There will be dates available for you to select from.  If you have been rescheduled to this particular date you will need to call the jury commission.  An email will be sent to your personal email address as to the decision of the jury commission.

Excusal:  Jurors are not excused TOTALLY from jury service.  You must call the jury commission if you feel you cannot serve.  At that time a judge will make that determination.
